What is a Diabetes Education Program?

Diabetes Education Programs (DEPs) offer group and individual-based sessions to provide individuals with the skills to self-manage their diabetes.

A nurse and dietitian help develop a plan for individuals and support them in managing their diabetes symptoms.

The DEP also provides a variety of resources available within the community.

Helps prevent the risk and onset of diabetes complications
Educates individuals with prediabetes or diabetes on self-management skills to improve health and wellbeing through individualized goal planning
Helps individuals with diabetes achieve target blood glucose levels
Provides medication counseling, including insulin initiation
Reduces the risk of hospitalization by enhancing patients’ knowledge from prediabetes, newly diagnosed, to a patient with a progressing disease state
Educates individuals with diabetes on the importance of having their quarterly and yearly visits with their primary care providers
