What is a Diabetes Education Program?

Diabetes Education Programs (DEPs) offer group and individual-based sessions to provide individuals with the skills to self-manage their diabetes.

A nurse and dietitian help develop a plan for individuals and support them in managing their diabetes symptoms.

The DEP also provides a variety of resources available within the community.

How will Diabetes Education help me?


Diabetes education:

  • Helps prevent the risk and onset of diabetes complications
  • Educates individuals with prediabetes or diabetes on self-management skills to improve health and wellbeing through individualized goal planning
  • Helps individuals with diabetes achieve target blood glucose levels
  • Provides medication counseling, including insulin initiation
  • Reduces the risk of hospitalization by enhancing patients’ knowledge from prediabetes, newly diagnosed, to a patient with a progressing disease state
  • Educates individuals with diabetes on the importance of having their quarterly and yearly visits with their primary care providers
