Service Description:

Supports for people living with prediabetes or diabetes * one-on-one or group consultation on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices * registered nurse and dietitian on staff
- gestational diabetes
- weight management
- pre-existing diabetes with pregnancy
- pump therapy

South Markham Chinese Clinics - Aaniin Community Centre - 5665 14th Avenue * clinic runs Tues 9am-5pm and Thurs from 1pm-9pm, offering basic diabetes group education, followed up with individual assessment * conduct counseling/assessments and book appointments in Cantonese and Mandarin
South Markham South Asian Clinics - Aaniin Community Centre - 5665 14th Avenue * for people who speak South Asian languages including Hindu, Tamil, Punjabi, Gujarati and Urdu * clinic run Wed 10am-6pm and Fri 8am-4pm * individual counselling and group sessions are offered at these sites Wednesday and alternating Friday
