Service Description:
|
Support for those caring for a loved one at home or from afar:- develop and evolve your personalized care plan with you
- provide in-home social visits from specially trained volunteers
- participate in individual counselling and/or support groups (based on community need)
- assist you with referrals and in connecting with other community caregiver services
- support you in the transition to grief and bereavement supports
- participate in complementary therapies and wellness activities (e.g. yoga, reiki, etc)
- participate in educational workshops (based on community need)
