Services include:
- 8-10 brief one to one and/group counselling services
- co-ordinated care management
- goal-focused plans based on the recovery and harm reduction models
- extensive internal and external community referrals

Free * no Ontario Health Insurance required for general services

Call or visit * referrals should be made by family physician * appointments required for some programs. Eligibility = OHIP or FSH card and 18+
