Mental health and substance use counseling as well as chronic disease monitoring, weight management and nutrition education
Services include:
- 8-10 brief one to one and/group counselling services
- co-ordinated care management
- goal-focused plans based on the recovery and harm reduction models
- extensive internal and external community referrals
