Service Description:
|
Adult day program * social, recreational, and therapeutic activities for older adults with Alzheimer's Disease of related dementias in a group setting for part of the day * helps to support family caregivers who provide day-to-day careService for an additional fee:- hair care - currently on HOLD
- foot care (every 6 weeks) - currently on HOLD
Transportation: Assistance for coordination with York Regional
Transit application can be provided * all transportation services must
be "hand-to-hand" (caregiver must be able to meet transit)
Nutrition: Hot lunch and two snacks * diabetic diet can be accommodated * participants must be able to feed themselves
Mobility Assistance: Participants must be able to transfer
themselves with minimal assistance * limited number of mobility aids
allowed due to safety
Incontinence: Accommodations can only be made for bladder
incontinence * assistance with monitoring and toileting * not able to
accommodate bowel incontinence
Medication Management: Program managers and senior program
workers can supervise taking of medications * clients are encouraged to
take medications before and after program * no injections * blister pack
required

$25 per day * subsidy available for eligible participants * additional fee for transportation
