Service Description:

Partnership between the March of Dimes Canada and Mackenzie Health facilitates support to individuals who live with the effects of an acquired brain injury * team includes Behaviour Consultants, Case Managers and Rehabilitation Workers

Services available:

case coordination and comprehensive functional assessment to evaluate the effects of the acquired brain injury on the individual and those who support them
individualized service/treatment plan, developed with recommendations and community support to promote the acquisitions of skills and strategies
plan which will enable persons with an acquired brain injury to live with maximal fulfillment in a state of dignity within the community

Self referral, professionals, family members or others with client consent

Youth 16 years and older, Young Adults and Adults with brain injury who reside in Simcoe County, Muskoka or York Region

English ; French
