Service Description:

Community Paramedicine programs support vulnerable populations within the community outside of regular 9-1-1 operations, services include:
CP@Clinic

seniors wellness clinic (location varies based on need)

Community Paramedicine and Outreach Team (CPORT)

social and healthcare clinic provided to persons experiencing homelessness

Integrated Primary Care programs

in-home supports and primary care for seniors with chronic conditions and who cannot not attend other forms of primary care

Home Visit program

supporting individuals within the community who rely on emergency services and healthcare to support health and social needs
specialized services, community partnerships and referral pathways to enable assessment, assistance and advocacy for patients and caregivers within the community
