Service Description:
|
Short term psychogeriatric services to older adults and their care partners, with mental health and/or dementia concerns * visits by community clinicians (Social Work, Occupational Therapist, Registered Nurse and Nurse
Practitioner) to complete a comprehensive psychogeriatric assessment,
treatment planning, advocacy, education and referrals to other services as
required * home visits, via Ontario Telemedicine Network (OTN),
with Geriatric Psychiatry * follow up as required
