Service Description:
|
A teaching, self-management program for patients with chronic heart failure (CHF)and/or chronic obstructive pulmonary disease (COPD) * patients are loaned a tablet, blood pressure cuff, oxygen monitor and scale * Telehomecare nurses teach patients how to check their weight and vitals and provide bi-weekly health coaching by telephone * designed to help patients achieve the best possible quality of life by learning to manage their condition
