Service Description: Supports for people living with prediabetes and diabetes or at risk of developing type 2 diabetes * one-on-one counseling or group consultation on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices * nurse diabetes educator, dietitian diabetes educator and social worker on staff

  • support groups and Saturday group sessions
  • referrals for community exercise programs
  • community mobile clinics
  • outreach workshops

Application: Medical referral required
Eligibility / Target Population: Adults 18 years and older with pre-diabetes and type 2 diabetes * undiagnosed at risk people
