Service Description: Social, recreational, and therapeutic activities, for people with an acquired brain injury (non-progressive) in a group setting for part of the day * helps to support family caregivers who provide day-to-day care


Service Details - Adult Day Program
Transportation Provided by York Region Transit * Mobility Plus * some assistance in arranging transportation may be available based on resources
Nutrition Beverages hot and cold provided * client brings own lunch and snacks
Mobility Assistance Program is wheelchair accessible and is able to accommodate a variety of mobility aids or equipment * must be able to transfer independently or with one person assist for toileting or transitions within program
Incontinence Clients must be continent of bowel/bladder or be able to manage independently or with one person assist
Medication Management Client must be independent taking medication with staff reminders only

Fees: $20 per day * sliding scale based on income * subsidy available
