Service Description:
|
Offers a variety of quality activities and services, in a safe and supportive environment, to individuals living in the community who are physically frail, have a mild to moderate cognitive impairment or are socially isolated * each day the participants have the opportunity to take part in a variety of social, recreational, therapeutic and wellness activities
Components of the service include respite/relief for caregivers, planned activities, nutritious meals, assistance with activities of daily living and health promotion * participants must be comfortable in a group setting * participants must not require heavy physical care or have severe cognitive impairment
Transportation:
- participant or their families are to arrange transportation to/from day program independently
- participants arrive with caregivers, Wheel Trans or agency
Nutrition:- nutritious meals and snacks provided
- therapeutic diets available (diabetic, modified texture)
- must be able to feed themselves
Medication:
- participants must be self medicating
- medication reminders available
Mobility:- must be able to weight bear and require no more than one-person assistance with transfers and ambulation
- unable to accommodate two person transfers
- unable to accommodate electric wheelchairs or scooters due to facility space
Incontinence:
- limited toileting assistance available
- urinary incontinence must be self managed
- bowel incontinence is unable to be supported
