Service Description: Health care services offered at home, at school, and in the community, as well as access to long-term care and supportive living programs * care coordinators work with people in the community or patients leaving the hospital to determine what help is needed and arrange for care *provides information about other community services and refers when appropriate

Children's Health Services - Special health services for children in both the home and school environment * supports children when health care provision is essential to a student’s participation and development in school programs * services are provided within public, private or home-based schools

Mental Health and Addictions Nurses - Assessments, interventions and support to students and families with mental health and or addiction issues * consultation to school staff in regards to mental health and/or addiction issues as appropriate

Mental Health Program - coordinates specialized health care services for individuals diagnosed with psychiatric disorders * services include psychiatric nursing, specialized mental health occupational therapy, and social work services * mental health care coordinators conduct assessments, coordinate services, develop care plans, and set up links with mental health agencies and community resources that can assist patients over the longer term

Placement Services - Determine eligibility for admission to long-term care facilities and prioritize and manage the admission process * support and information is provided to applicants and their families prior to and during the admission process * determines eligibility for Short Stay respite in a long term care facility

Community Nursing Clinics - Nursing services for ambulatory patients at clinics located in the community * attending these clinics is by appointment only 

Telehomecare - a teaching, self-management program for patients with chronic heart failure (CFH) and/or chronic obstructive pulmonary disease (COPD) * patients are loaned a tablet, blood pressure cuff, oxygen monitor and scale * Telehomecare nurses teach patients how to check their weight and vitals and provide bi-weekly health coaching by telephone * designed to help patients achieve the best possible quality of life by learning to manage their condition

Adult Day Programs - Programs that promote wellness, dignity, and enhance quality of life in a social setting, and provide a much-needed break for persons providing care and support at home

Health Care Connect
- Refers people who do not have a doctor to a family health care provider who may be accepting new patients * once registered, program participants are assigned to a Care Connector * Care Connectors work with family doctors and nurse practitioners who are accepting new patients

Exercise and Falls Prevention Classes - Exercise classes focus on improving and maintaining functional performance through strength and balance to prevent injury and falls * falls prevention classes provide instruction on how to reduce the risk of falling * classes are provided by Better Living Health and Community Services, Carefirst Seniors and Circle of Care
