North York General - Two diabetes centres to serve you

North York General Hospital offers two centres to serve people with diabetes:

Diabetes Education Centre
 
Did you know the Diabetes Education Centre (DEC) at the Branson Ambulatory Care Centre offers group programs and individual counselling for people with diabetes and their families at all ages and life stages? The DEC has been open for over 30 years and is an active centre with close to 11,000 patient encounters every year.

The interprofessional team includes nurses, dietitians, social workers and endocrinologists who specialize in paediatrics, diabetes in pregnancy, diabetes management and education.

Centre for Complex Diabetes Care
 
North York General  is one of six centres across Ontario to have received funding from the Ontario Diabetes Strategy to host a Centre for Complex Diabetes Care (CCDC). The CCDC is a regional program, serving all of the Central Local Health Integration Network.

While most patients usually manage quite well with standard levels of care, the CCDC provides comprehensive care for patients with complex diabetes needs. Care is provided over a shorter-term, is patient-centered, goal specific, and allows for frequent follow-up.

The CCDC assists with coordinating care among health care providers to maintain a patient's continuity of care and to avoid duplication of service.
