Contact the Ontario Health atHome Central area. The best advice is to register with Ontario Health atHome as they are the first point of contact for home care services. If you are eligible, services may be available at no cost.

Do you need other palliative care help (at no cost) such as information on navigating the health system, respite or volunteer help? There are community support services (at no or very low cost) that offer practical non- medical help, such as meals on wheels or transportation. As well as the low to no-cost services, you can buy extra help to stay at home.

No longer able to stay at home?
Community-based services may enable you to stay in your home and community longer than you thought possible. However, when your care needs can no longer be met in the community, there are other options such as residential hospice, palliative care units in hospitals and long term care homes. Talking to your physician, nurse, the Ontario Health atHome Care Coordinator, or other health care team members can help you with these options, assess your specific needs and provide more information.
