Purpose |
Diabetes management and education support:
- Education on diabetes and healthy living
- Assists patients in developing plans to help minimize symptoms and prevent diabetes complications
- Some DEPs offer education and management support to women with gestational diabetes

The workshop is an excellent program to enhance the information provided at your local DEP but it’s not meant to replace it.
Learn:
- Problem solving and goal setting skills
- Strategies to better manage symptoms
|
Provides comprehensive care for patients with complex diabetes needs. Care is provided over a shorter-term, is patient-centered, goal specific and allows for frequent follow-up. |
Participation Criteria |
Physician referral or self-referral.
18 years or older with any of the following: Type 1, Type 2, Gestational Diabetes and Prediabetes.
Pediatric (17 years and under) Diabetes Education Programs in Central LHIN include:
- Markham Stouffville Hospital

A physician referral is not required.
Participation is open to anyone who has one or more chronic conditions or is a support person for someone with a chronic illness. |
Physician referral.
Patients seen in the CCDC include adults 18 years of age and older who are experiencing one or more of the following:
- Unmanaged diabetes complications
- Comorbidities which impact glycemic control
- Barriers in accessing health care: e.g. serious mental illness/poor mobility/frail elderly
- Recurrent hospitalization/Emergency Room visits
- Complexities requiring case management

A team of health care professionals which may include:
- Diabetes Educators (Registered Nurses and Registered Dietitians)

Peer leaders offering no specific medical advice. |

Length/Group or Individual
|
Group and individual-based sessions. |
Six week workshop; group setting. |
Individual assessment and treatment. As patients meet their goals and their condition stabilizes, they are transitioned to other services such as a DEP.
