Service Description: Supports for older adults and individuals with disabilities * helps people remain independent in their own homes for as long as possible * referral to other community services

Services include:
  • in-home health services and community based programs to individuals of all ages
  • long-term care program includes five Adult Day Programs - Barrie, Collingwood, Orillia, Beeton and Bracebridge
  • in-home respite and enrichment program that is available to individuals with a diagnosis of Alzheimer Disease or related dementia
  • coordinate Home At Last (HAL) Program and SMART Seniors Maintaining Active Roles Together Program

VON Canada Simcoe County Site is a member of the Simcoe Muskoka Integrated Fall Strategy (SMIFS) and member of the North Simcoe Muskoka Community Support Services Coalition
