Service Description: A comprehensive geriatric assessment and treatment plan for older adults who are homebound due to physical, cognitive and/or psychiatric conditions * brings together existing primary, community, speciality and acute care resources into one team to care for the most complex clients * three streams (shared care, primary care and consultation) available to the clients

Fees: Most services covered by OHIP * varies by program
Application: Family doctor must complete and fax the Baycrest referral form.
Eligibility / Target Population: Adults aged 65 and over who are homebound due to complex physical, cognitive and/or psychiatric conditions
Languages: English
Language Notes: Although some staff speak other languages, the program is primarily English based * translation can often be arranged when required
