Service Description: Two bedroom apartment unit, for older adults needing short stay accommodation (5-30 days) with personal support * a PSW is assigned to the apartment 14 hours a day between 8am-10pm to assist with activities of daily living * overnight visits provided on a scheduled basis * clients who come to stay are transitioning out of the hospital, are waiting in the community for a Long Term Care Bed or are community clients requiring short term personal care support * accommodation is not a locked-in unit * priority given to Individuals residing in Home and Community Care Support Services Central West

Eligible tenants living in Richview Residence buildings may be offered other services as an alternate to the transitional bed
  • assisted living/supportive housing for personal care services
  • congregate dining

Eligible tenants living in Widdicombe Hill/Blvd buildings may be offered other services as an alternate to the transitional bed
  • assisted living/supportive housing for personal care services

Fees: Care Centre (transitional care beds): $40 per day includes accommodation, meals, and PSW supports
Application: Information and intake by telephone is available Mon-Fri 9:30am-4:30pm
A message can be left at anytime and your call will be returned the next business day

Eligibility requirement related to mobility, cognition and personal care needs
InterRAI CHA assessment tool used to determine service plans
Languages: English * German * Hindi * Sinhalese * Spanish * Tagalog
Language Notes: Staff speak and write English * limited availability for other languages, based on staffing
