Service Description:

12 week educational and exercise program for patients with cardiovascular diseases such as heart attacks, cardiac disease, stroke. and transient ischemic attack * program includes weekly exercise and education sessions and Physiotherapist assessment

Must be referred by a Physician or Nurse Practitioner from Outpatient Stroke Prevention or Cardiac clinics at Oak Valley Health - Markham Stouffville Hospital
