Service Description:

Program is a component of outpatient services * goals are to increase patients' sense of control over illness and prevent hospitalization

Services available:

support for schizophrenia or other psychotic disorders
administration of medication and monitoring
education and instruction
mental health education
collaboration with community partners

Referral by medical or psychiatrist

Eligibility / Target Population:

Youth 16 years and older * Adults * that have a formal diagnosis schizophrenia or schizoaffective illness * must have family physician (GP) in Simcoe County
