Service Description:

This outpatient program is designed to provide assessment and treatment planning for individuals requiring long-acting injectable medication and monitoring. The program includes individual, group and family education, as well as medicine administration and monitoring.

Application:

Clients referred must have a psychiatrist with admitting privileges at Southlake and/or have affiliation with the clinic.
Each referral must be accompanied by a completed Adult Outpatient Referral Form. Call the clinic to request a form.
