Service Description: The Integrated Psycho-geriatric Outreach Program (IPOP) provides comprehensive psychogeriatric assessments by a community clinician consulting with a psychiatrist or geriatrician in the client's home. This program looks to promote the health, well-being, safety and independence of older adults living in the community with mental health and dementia related concerns. The services offered by this program include a comprehensive psycho-geriatric assessment, treatment planning consultation and follow-up. The IPOP team includes a community psychogeriatric clinician, and a consulting psychiatrist.

Application: Referrals are accepted from physicians, community organizations or self-referrals.
