Service Description:

Day program for older adults * social and wellness activities, companionship, nutritious snacks, and lunch * respite for caregivers * virtual programs available
Activities include:- exercise
- mental stimulation
- crafts
- relaxation programs
- entertainment
- health promotion
- disease prevention
Transportation - Transportation $5.20 one way if using CHATS * family can choose to arrange private pick up and drop off
Nutrition - Nutritious snacks and lunch provided
Mobility Assistance - Person requires minimal assistance with activities of daily living (meal, toileting and transfers – maximum one person transfer)
Incontinence - Person requires minimal assistance with activities of daily living (meal, toileting and transfers – maximum one person transfer)
Medication Management - Personal Support Worker administered medication program with a recognized medication compliance system (pre-poured system – e.g. blister package)
