Service Description: Offers free individual counselling and group education classes for people with Type 2 diabetes and pre-diabetes * program also offers free Teleophthalmology services including free eye exams and retinal screening services for individuals with Type 2 Diabetes  * diabetes educators include registered nurses and registered dieticians 

Fees: Free * no Ontario Health Insurance required for general services
Application: Self, doctor or health care provider referral * complete referral form located on website
Eligibility / Target Population: Adults 18 years and older with pre-diabetes and type 2 diabetes
