Service Description: Medical assistance in dying is available for adults residing in Canada who have a grievous and irremediable medical condition * under the Criminal Code of Canada permits this to take place only under very specific circumstances and rules * drugs that end life are either directly administered by the physician or nurse practitioner or prescribed and then taken by the patient themselves

Patients should discuss with their physicians or nurse practitioner for information * physicians and nurse practitioners are the ones who provide medical assistance in dying services

Application: Informed consent must be given (includes knowledge of and understanding of medical diagnosis, available forms of treatment, available options, including palliative care), a written request must be submitted, medical assessments are required and a ten-day waiting period is required * additional provincial requirements may be necessary
Eligibility / Target Population: To be eligible:
  • adult 18 years and older, mentally competent and eligible for provincial funded health coverage
  • have a grievous and irremediable medical condition
  • make a voluntary request for medical assistance in dying (includes knowledge of and understanding of medical diagnosis, available forms of treatment, available options, including palliative care)
  • give informed consent to receive medical assistance in dying; written request must be submitted, medical assessments are required and a ten-day waiting period is required
