Service Description: Provides education and awareness of how to prevent a fall. Workshop was created and is delivered by 5 different health professionals (RN, Social Worker, Pharmacist, Dietician, Chiropodist)

Patients complete a falls risk assessment and based on scores and interest, they are directed to one of the health professionals for specific 1-1 care (ie. med review, senior service navigation, foot exam etc)

Presently workshop is open to rostered patients of the FHO connected with the FHT, with plans to open up to the community as part of Patients First Act
