Service Description:

Team works with residents, family, staff and other health care professionals at individual long-term care homes to:

manage/treat challenging behaviours and mental health / psychiatry concerns at site
avoid unnecessary visits to the Emergency Department
ensure safe transfers to and from the hospital, when appropriate
enhance support and build knowledge capacity, as needed

Provide the following specialized services for residents: assessment, consultation/ Treatment and short-term follow-up

The team is made up of Advanced Practice Nurse, Nurse Clinicians with expertise in the area and Psychogeriatrician (a specially-trained doctor in this area)
