Service Description:
|
Provides support to racialized individuals to transition from persistent homelessness to living independently within a community. Provide an Intensive Case Management (ICM) service for people from racialized communities living with mental illness and homelessness based on the Housing First model of service.

Clients looking to access this program must contact The Access Point. Referrals are accepted from family members, friends, self, community agencies, physicians, professionals, lawyers, etc.
