Service Description:

Offers a range of support and services for transitional aged youth (ages 15-24) with mental health and/or substance abuse issues. It is offered in collaboration with the Black Creek Community Health Centre.

Clients looking to access this program must contact The Access Point. Referrals are accepted from family members, friends, self, community agencies, physicians, professionals, lawyers, etc.
