Medical health care team focused on family and community health * coordinates patient care in order to assess and treat injuries or illness * team includes family doctors, nurse practitioners, nurses, child and youth social worker, registered dietitians, addiction counselor, case worker, chiropodist and pharmacists

- Craving Change - diet and nutrition
- mental health promotion - one-on-one and group therapy
- heart health program
- CBT (Cognitive Behavior Therapy) - depression and anxiety, and insomnia
- Smoking Cessation Program
- diabetes education program
- NP Led homebound program
