Service Description:

Home-centered hospice program designed to provide psychosocial, emotional and practical service to the Jewish community * services include:- counselling
- care planning
- resource identification
- advocacy
- chaplaincy
- volunteer support to clients and their families
- counselling for children and teens coping with parent's illness
- counselling support for caregivers, caregiver relief provided through trained volunteers
- bereavement support in individual and group formats
Part of a larger Jewish multi-service agency allowing clients to access numerous levels of resources/services as appropriate * cultural, religious, spiritual expertise to assess and support client * staffed by professionals including social workers, Rabbi and Volunteer Service Coordinator in partnership with Circle of Care * for clients and families who identify as Jewish * HPCO accredited program
