Service Description: Culturally specific integrated program for frail and elderly individuals or those living with mild dementia/cognitive impairment * clients with mild responsive behaviours must be manageable in a group setting

Transportation: agency transportation available at additional cost or Mobility Plus can be arranged by the family

Nutrition: two snacks per day * catered hot and cold lunch * food cost included in program price

Mobility Assistance: minimal assistance is provided * must be able to move independently with or without aids * person must be independent with feeding

Incontinence: offer one person assistance for mobility and toileting but no bowel incontinence accepted * urinary incontinence must be self-managed

Medication Management: medication reminders are given in a blister pack * clients are encouraged to take medications before and after program * cannot administer insulin or injections

Fees: $40 daily ($32 daily if 2 or more days per week attendance) * transportation fee $19-$24 per round trip

Eligibility / Target Population: Older adults aged 55 or above with disabilities, who need physical, social, or emotional supports, who experience social isolation with limited family and/or peer support * will not accept clients with behavioral issues
