Service Description: Consultation by a specialized geriatric nurse within a hospital emergency room to provide assessment, diagnosis, identification of "frail and at risk" elderly, and to initiate appropriate treatment in the hospital or community

Fees: None
Eligibility / Target Population: Seniors who have one or more of the following: live alone, difficulty walking, functional decline, frequent falls, 5 or more medications, history of cognitive impairment, suspected elder abuse or neglect, ED use in previous 30 days, as well as an RN or any other emergency's team member professional recommendation
Languages: English * French * Spanish
Language Notes: Interpretation services available including American Sign Language (ASL)
