Supports for older adults * helps older adults remain independent in their own homes for as long as possible
Caregiver support and other homemaking services:- Adult day programs
- care coordination
- client intervention and assistance
- Meals on Wheels * frozen meals delivered once a week
- homemaking
- assisted living services * 24 hour in home support for high risk seniors
- transportation services * door-to-door assisted transportation for seniors 55 years and older, and adults with disabilities who are not eligible for Wheel-Trans * services offered through iRIDE PLUS, click HERE for more information
Healthier Living Centre * independent seniors day program:
- social, recreation and education programs
- congregate dining
Home At Last * hospital discharge assistance
