Service Description:

Conducting assessments, planning, coordinating services and ongoing monitoring to meet the needs, goals and expected outcomes for those living with Alzheimer's disease or other dementias and their caregivers * provide education around the disease process * help connect to other services in the community * one-on-one counselling to provide emotional support to caregivers * First Link Caregiver education series run across the region

Meet with your peer virtually to share information on coping with, and caring for people diagnosed with Alzheimer's disease and other related dementias using Zoom * Zoom meeting ID and password will be provided upon confirmation or registration. Contact 1-888-414-5550 if you are new to this. |

Disease specific eligibilty Alzheimer's disease and other dementias (progressive, irreversible) e.g. Vascular dementia, Frontal Temporal Lobe Dementia, Lewy Body Dementia, etc.
