Service Description:
|
In-person adult day program * social, recreational, and therapeutic activities for post-stroke, frail, and older adults living with Alzheimer's disease in a group setting for part of the day * helps to support family caregivers who provide day-to-day care
Virtual services include:
- day programing
- music sessions
- exercise sessions
- video chat
- one-on-one sessions
Service Details - Adult Day Program (In-person)
Transportation: transportation can be provided for members who live within the service boundaries
Nutrition: light breakfast * snacks and nutritious Chinese-style hot lunch are provided * special diets (vegetarian, minced, pureed, etc) can be arranged upon request
Mobility Assistance: assistance can be provided to clients who require one-person transfer
Incontinence: assistance in toileting and diaper-changing can be provided
Medication Management: medication-reminding service can be provided
Virtual Day Programing and Services: available on scheduled days
Technology Support: electronic devices can be provided to clients who lack devices to access to virtual services

$32 per regular hour in-person attendance * subsidy available for eligible participants * additional fee for transportation and extended hours
