Health centre focused on family and community health * health professionals assess and treat non-life-threatening injuries or illnesses * referrals to other local health services and personal development groups * team includes family doctors, nurse practitioners, registered practical nurse, dietitian, physiotherapist, chiropodist (foot specialist), and a social worker
Community-led health teams develop programs to improve the social supports and conditions that affect long-term health- counselling
- prenatal and postnatal
- treatment of function and mobility issues
- healthy relationship, bereavement, stress management support
- nutrition education
- treatment and management of chronic diseases like heart disease, diabetes, chronic obstructive pulmonary disease (COPD), depression, and arthritis
