Service Description:
|
Provides seniors with assessments, treatment planning, education and referrals to community resources by a geriatric psychiatrist * this visit is provided to home bound seniors with mental health issues who are not actively being treated by a psychiatrist and cannot get into clinic

Application:

Medical referral from a primary care provider required * frail health and mental health concerns * must be unable to travel to a clinic and require a home visit by a mental health professional
