Service Description: Offers six week interactive workshops for people with chronic health conditions * topics include physical activity, healthy eating, medication management, future planning, communicating with your health care provider, action planning, depression management, and pain management 

Fees: No cost
Application: Residing in NYGH catchment area for all services except the Memory Disorders Clinic which services patients 60 years and older
Eligibility / Target Population: Adults with chronic health problems
