Service Description: Day program for adults * social, recreational and therapeutic group activities for people with Alzheimer's disease or associated dementia * helps caregivers providing day-to-day care
  • daily exercise program
  • meals
  • gardening in outdoor secure patio
  • games
  • arts and crafts
  • musical entertainment
  • baking/cooking

Transportation: Offer two options * 1)York Regional Transit; hand to hand transportation, caregiver or person must be present to meet client * York regional Transit is a contracted service provider at a minimal cost to client * 2) Caregiver/family can provide transportation

Nutrition: Light breakfast, lunch and nutritious snacks daily

Mobility Assistance: Minimal assistance and must be able to ambulate independently * walkers and wheelchairs (individuals able to bear weight)

Incontinence: Program is able to provide assistance for clients with manageable bladder incontinence * not able to manage bowel incontinence

Medication Management: Medication cueing provided * all medication is secure and staff provide a reminder * medication must be brought in a pharmacy created blister pack and include only required medication for program hours with appropriate information labelled

Fees: $25.49 per day * subsidy available for eligible participants
