Service Description: Adult day program * social and wellness activities for older adults in a group setting for part of the day * helps to support family caregivers who provide day-to-day care* virtual programs available

Services include:
  • lunch and nutritious snacks
  • exercise
  • mental stimulation
  • crafts
  • relaxation programs
  • entertainment
  • health promotion
  • disease prevention

Service Details - Adult Day Program

Transportation:
$5.20 one way if using CHATS * family can choose to arrange private pick up and drop off * YRT Mobility Plus

Nutrition: hot lunch meal plus refreshments

Mobility Assistance Program: is wheelchair accessible and is able to accommodate a variety of mobility aids/and or equipment * must be able to transfer independently or with one person assist maximum

Incontinence: client must be continent of bowel/bladder or be able to manage independently or with one person assist

Medication Management: client must be independent taking medication with staff reminders only * no nursing staff on site

Fees: $30.50 per day * subsidy available for eligible participants * additional fee for transportation if available
